My interpretation was that he's the third oldest, and Colt and Akita are still the youngest there. Though his birth did indeed deter his parents for a while. His birth, and Negev's weird biomechanical vanishing twin/fetus in fetu thing state developing enough for her to emerge from a growth on Beretta's core as a very hungry caterpillar who turned into a headcrab core after collecting enough matter.
My take is the first four were born through weird Solverpede shit where Uzi used her and N's data to generate their cores inside her and birth them through her canal. Geist and Negev scared them off from trying again, and a few years later they figured out how to have children the normal way and that's where Colt came from... then came Akita's miraculous conception.

Keeter's original concept has been putting around in Blue's art since episode 4 dropped (that red nightmare-angel from the "moth-er" image on the booru) and Colt's been the subject of a fic since episode 6. Mac might technically be older than at least Colt (at least in concept), but I'm pretty sure his whole revival as a non-incestuous being (honestly he never even had a rape green, just an attempted one, sad side effect of being conceived of during Berettabuse) came way after.
